Conference Room Reservations

Please check room availability on Oracle Calendar before requesting a specific room.  See Humanities Conference Room Reservation Protocol for instructions and more information including details on conference room space and a summary of seating styles and room equipment. Please contact Humanities Academic Service Center (HAS) if you have any questions.

FOAPAL Account Information
We require FOAPAL account information for reservations in case there is any damage or if the room needs to be cleaned or reorganized. Trash removal is the responsibility of the event coordinator.  Please return the tables and chairs to the original configuration before vacating and close windows and doors. The following charges may apply: cleaning ($25.50/hour, one hour minimum); lost key fee ($75); damaged key ($4.75); damages to furniture/equipment (actual replacement cost); conference calls (actual cost).
Weekend Event?

If your event is scheduled M-F before 8 AM or after 5 PM, Please contact Tony Grant(tegrant@ucsc.edu, ext. 9-4710) 24 hours in advance for a key.  If this is a weekend event, please contact Tony Grant (tegrant@ucsc.edu, ext. 9-4710) at least one week in advance for room key.

Catered Event?
If so, who will be catering?

Please make sure you have completed all fields in this form as they are required. Don't forget to print this form for your record before you submit it. Just select "Print" from the File menu in your browser.

The Humanities Division reserves the right to reassign or cancel room reservations if necessary.
